You have to withdraw from National Health Insurance if you have joined the social insurance of your company/employer.

Specific procedures are required, if you meet either one of the following criteria:

・ You have started working at a company, and you have joined a workplace health insurance.

・ You have joined your family member’s workplace health insurance.

 

How to withdraw from the National Health Insurance

● By post

Enclose the following documents in an envelope and send them to Kokubunji City: Address:Hoken-nenkin-ka 1-6-1 Tokura, Kokubunji-shi, 185-8501

・ Fill in the National Health Insurance Insured Person Qualification Change Notice form.

(You can download the form from Page No. 1004490 of the Kokubunji City Website.)

・ The original National Health Insurance Card(s) of all family members who will withdraw from the National Health Insurance(Photocopies are not accepted).

・Photocopies of the health insurance card(s) issued by the company/employer of all family members who will withdraw from the National Health Insurance.

 

● Submitting in person

Bring and submit the following documents to Hoken-nenkin-ka (located in the 1st Building of Kokubunji City Hall).

・ The original National Health Insurance Card(s) of all family members who will withdraw from the National Health Insurance(Photocopies are not accepted).

・ The original health insurance card(s) issued by the employer/company of all family members who will withdraw from the National Health Insurance.

 

If you have an insured period of National Health Insurance on/after April 1st, you must pay the insurance tax for the period until you have withdrawn from the National Health Insurance. We will send the payment slip in July by post. Please pay the bill without fail.
